The caregiver signs up.
The caregiver authorizes the records search, completes intake and reserves a Quest appointment in case a test is needed. No visit if records clear them; one visit if a test is needed. Sign-up is available in English, Spanish and other languages. Caregivers who pay see both prices before adding a card.
We look for prior records. A physician reviews.
We request lab and clinic records. A California-licensed physician reviews them alongside the caregiver’s intake answers.
No new test unless it is needed.
If records support clearance, we cancel the Quest appointment and notify the caregiver. If more is needed, we coordinate testing or follow-up after physician review.
Your team receives the completed TB file.

Standard agency-paid rates are shown below. Your caregiver-paid offer has its own prices: first caregiver $0 from records or $69 with a new Quest test; then $45 from records (through March 20, 2027) or $99 with a new Quest test.

The two prices are alternatives, not added together. Booking alone does not trigger the new-test price. Outside follow-up, including a chest X-ray, is separate. Program-specific billing rules apply.

Why a skin test can mean extra follow-up
A past TB vaccine can make a skin test look positive even without TB infection. That can lead to follow-up, including a chest X-ray.
How the blood test helps
The blood test is not affected by that vaccine. With a negative result and no clinical concerns, clearance can proceed without a routine chest X-ray.
Positive results or clinical concerns still need follow-up.
An indeterminate result may need another test. Your clinical team explains and coordinates the next step.
Know what “Clear to Start” means.
“Clear to Start” means the TB certificate is complete. Your agency checks other hiring requirements, decides whether to accept the file and makes the hiring decision.
While a case is open, including any follow-up, your office sees the case status and the current step, not the caregiver’s intake form.
Once the file is complete, authorized users receive the signed TB certificate and the supporting TB result.
What can the physician use?
Where do the records come from?
We request records directly from the lab or clinic where your caregiver was tested, with their permission.
Can you use a prior skin test or blood test?
Yes. We can review records from either test. If the records and health questionnaire support clearance, no new test is needed.
How recent does the record need to be?
We check the record’s date, result and supporting information against California’s requirements.

Transferring from another home care organization
A certificate documenting an examination within the past two years may qualify. The prior agency can also verify its certificate and send a copy before employment begins.
Other new hires
The examination must fall within 90 days before employment or seven days after. Your agency must have clearance documentation before the aide works.
A new signature does not reset the original test date.
Does everyone need a doctor’s appointment?
Usually, no separate office or video visit is needed. The physician reviews the records and health questionnaire first. If a visit or more information is needed, we explain the next step.
Who signs the certificate?
A California-licensed physician signs after clearance. Their name appears on the certificate.
How soon is the certificate ready?
Timing depends on whether we use records or a new test:
- Lab records usually arrive the same day.
- Clinic records usually take two to three days to retrieve.
- New blood-test results take about 48 hours.
The physician then completes the review. Missing information or follow-up can add time.
What happens after a positive result?
We coordinate an X-ray appointment and follow-up. Once the physician confirms clearance, we deliver the signed TB certificate.
ClearToStart does not cover the cost of the X-ray or other outside care.
New positive result: This does not by itself mean active TB.
Previous positive result: Tell us during sign-up. The physician reviews earlier results and follow-up records instead of routinely ordering another screening test.
Unclear result: Another test may be needed.
