For California home health agencies onboarding hires who need new personnel health paperwork

One hire. One Quest appointment. We handle the rest.

For RNs, LVNs, CHHAs, therapists and other employees with direct patient contact, the worker completes one short online health-and-duty form. When a new TB test is needed, the California physician orders one QuantiFERON blood test at Quest. When the required intake is complete and the TB result is negative, the result and doctor-signed assessment arrive together in your dashboard within 48 hours of the Quest visit.

Worker pays $0. No completed clearance file, no $99 invoice.

What your agency actually keeps

A physical and TB result are the starting pieces.

Your worker may already have a recent physical or TB result. Those records still need to be reviewed and returned as the completed health assessment your agency keeps.

The starting pieces

The worker's health information, physical record and TB result.

The finished file

One health assessment signed by a California doctor, with the TB paperwork and supporting result together.

What ClearToStart does

We collect the information, track the Quest result, get the doctor signature and put the completed file in your dashboard.

How it works

From referral to finished HHA file, in five steps

For your worker

Easy on the worker, by design.

One short online form

The worker completes the health-and-duty form online — then one Quest appointment when testing is needed, at a location that works for them.

Plain-language instructions

Every step is spelled out — where to go and what happens next.

A real person to reach

The worker's instructions include a direct support contact for questions.

What we take off your plate

One referral in. One finished HHA file back.

ClearToStart sends the instructions, collects the health-and-duty information, tracks the Quest result, gets the California ordering physician assessment completed and signed, and places the finished file in the secure dashboard.

One secure dashboard for the whole process.

When the completed health file is ready, users receive an email notification and sign in to view and download the Quest result and paperwork signed by a California doctor. The clinical result stays inside the dashboard - no loose paperwork moving between inboxes.

Create a registration link or QR code and send it directly to the worker.

Track

See which files are complete, which workers are waiting, and each worker's next testing date.

Add services when you need them

Keep the next testing date beside the file and activate automatic reminders before an update is due. OSHA hepatitis B support and flu-shot coordination can be added separately.

The one-hire pilot

Try it with one hire — $99

The pilot covers the online intake, a QuantiFERON TB blood test ordered by a California doctor when needed, one Quest collection, doctor review and signature, and dashboard delivery. The worker pays $0. We issue the $99 invoice only after the completed file signed by a California doctor is in your dashboard.

No completed clearance file, no $99 invoice.

Licensed providers make clinical decisions. Your agency decides whether to accept the file and hire the worker.

Questions

Frequently asked questions

Who is the HHA service for?
RNs, LVNs, CHHAs, therapists and other employees with direct patient contact who need a written health assessment and TB documentation.
What does the worker have to do?
The worker completes one short online health-and-duty form and one Quest appointment for QuantiFERON TB blood test ordered by a California doctor when a new TB test is needed.
How fast is the file ready?
When the required intake is complete and the result is negative, the Quest result and health assessment signed by a California doctor are placed in the dashboard together within 48 hours of the Quest visit.
What is included in the finished HHA file?
One secure, completed health file containing the California physician-signed health assessment, completed TB documentation, supporting Quest result and next testing date. A synthetic specimen is available at https://www.cleartostart.com/what-you-receive?lane=hha#specimen.
Can you use our agency's own health assessment form?
Tell us which form your agency uses. We review it during the walkthrough and confirm how the pilot will return the finished file. If your agency does not have a form, we use a California-compliant assessment.
Does the worker need a separate physician appointment?
Most workers do not need a separate video or office visit with the doctor. The California physician who orders the QuantiFERON test reviews the completed intake and Quest result and signs the assessment. If more information is needed, ClearToStart contacts the worker.
Who signs the paperwork?
A California-licensed physician reviews the Quest laboratory result and the worker's health form, then signs. The clinical program is led by Dr. Pavani Gunda, Global Chief Medical Officer. How clinical decisions are made
How is the file delivered?
Multiple authorized agency users receive an email notification and sign in to the ClearToStart dashboard to view and download the Quest result and signed assessment. The clinical result is not placed in the email.
When is the agency billed?
The worker pays $0. ClearToStart issues the $99 invoice only after the completed file signed by a California doctor is in the dashboard. If an agreed pilot document is missing, ClearToStart corrects the file before invoicing.
What happens if the result is positive?
ClearToStart contacts the worker, explains the next steps and arranges the required chest X-ray. We first look for a convenient free public-health resource. If paid imaging is needed, the agency is told about the cost before scheduling.
Can workers register through a link or QR code?
Yes. The agency can send a registration link or QR code directly to workers. ClearToStart then sends the assigned instructions and tracks the steps in the dashboard.
Can the dashboard help with renewals and flu shots?
Yes. The dashboard records testing dates and can automatically remind and engage workers when updates are due. For flu campaigns, ClearToStart can send workers an SMS directing them to a Walgreens appointment and track the vaccination record. Additional services are scoped separately. How renewals and reminders work
Does the agency retain the final decision?
Yes. Licensed providers make clinical decisions. Your agency decides whether to accept the file and hire the worker.
Does ClearToStart replace the agency process?
No. Keep your current process and evaluate ClearToStart with one selected hire.
Who is behind ClearToStart?
ClearToStart is operated by VitalCheck Wellness. VitalCheck has delivered employer health programs since 2015.
Where does the worker go when testing is needed?
The worker selects a Quest patient service center that works for them. The instructions explain how to schedule.
