California TB certificates are signed by Dr. Alexis Young through Ceregenesis Medical Corporation.

She practices through Ceregenesis Medical Corporation in Santa Monica, a California physician-owned professional medical corporation registered since 2021 and in good standing.
Check the physician named on your certificate using the Medical Board of California’s license lookup. You can verify the practice through the California Secretary of State.
3. The supporting result
Lab reports identify the laboratory and its CLIA number. Prior skin-test records identify the provider and the placement and reading details.
4. The service operator
ClearToStart is a VitalCheck Wellness service. VitalCheck has operated employer health programs since 2015; licensed providers make clinical decisions.
5. Security documentation
